Primary Health Care Forum 2018 celebrates eleventh year, explores health equity

The South East Local Health Integration Network’s (LHIN) Annual Primary Health Care Forum (PHCF) will take place on November 7, 2018, at the Ambassador Hotel and Conference Centre in Kingston.

The PHCF Planning Committee is pleased to announce this year’s theme is Intersections – Primary Health Care and Health Equity – a concept which sets the opportunity to increase awareness, and discuss policies and programs aimed at reducing barriers to good health.

Now in its eleventh year, this one-day networking and education event is largely geared towards primary care providers, but is also attended by those who represent many other health sectors.

Learning objectives for the event include:

  1. Creating a shared appreciation of how health equity is defines, as well as the various systemic barriers within the health care system that prevent all people from achieving good health;
  2. Better understand why health equity is relevant to primary care and why it is an important responsibility in today’s environment; and
  3. Exploring, through interactive learning sessions with colleagues, local initiatives and practical tools or approaches that help to decrease disparities.
